Warning Massive Spoiler Ahead in term of quest name....
Keep in mind that this guide does contain some spoilers if you haven't met all the factions yet, but I tried to keep it as vague as possible while still being clear on what to do.
First of all, it DOESN'T MATTER who you build the teleport platform for in the quest "The Molecular Level". The quest where can first make another faction an enemy is "Mass Fusion" which you is received from the Institute a good while after you build the teleport platform
You can finish all the Minute Men quests related to trophies fairly early on in the game and it doesn't make you enemies with any faction. The final Minute Men trophy is for the quest "Old Guns"
Shortly after meeting The Institute, you will be given the quest "Battle For Bunker Hill" from Father (the leader of The Institute). To remain allies with both The Institute and The Railroad, you will need to go to The Railroad HQ and inform them of Father's plan and then Kill the Courser during this quest. When you meet up with Father on the roof of the C.I.T Ruins at the end of the quest, pick the "Ambushed" dialogue option.
Shortly after completing "Battle For Bunker Hill", you will be given the quest "Mass Fusion" from Father. It is at this point where you'll want to make a save file to come back to and finish all The Brotherhood Of Steel quests because depending on who you side with you will either become enemies with The Institute or The Brotherhood Of Steel.
The next Institute quest after completing "Mass Fusion" is "Pinned", which has the potential to make you enemies with either The Minute Men or The Institute. Simply pick the Charisma dialogue options to have The Minute Men stand down and you'll remain allied with both factions.
Upon completing "Pinned", Father will give you a quest called "End Of The Line", which will make you enemies with The Railroad if you do what he asks. If you wait at The Institute for a bit, a Synth will come up to you and give you some information that will allow you to continue doing quests for The Railroad. You can actually quest with The Railroad all the way up to "Rockets' Red Glare" without making enemies with The Institute, which will net you every Railroad faction achievement. Make a save at this point before either continuing to work for The Railroad and obtaining the achievement "The Nuclear Option" or betraying them to work for The Institute and Father.
Now you have 3 choices on what to do next, continue working for The Railroad, work for The Institute to get the trophy "Nuclear Family", or load your save and work with The Brotherhood Of Steel to get all their trophies.
As a note, you can receive "The Nuclear Option" trophy from either The Minute Men, The Brotherhood Of Steel OR The Railroad, depending on who you side with. "Nuclear Family" is only gained by siding with The Institute.
