From my observation, depends on the time u left it open, if its just a short time u left the tab, it wont reload. But if left it for long time, it will reload. This also depends on apps usage as well, if many apps open outside, it will reload much more frequently. Iinm i think this got to do with memory management.

Yep, i notice this too since iOS 8.1 onwards if im not mistaken. Usually there is only one folder for photos and sometimes maybe two. But nowadays no, too many folders hard for me to identify which is which. I'm guessing now every apps using it's own folder or something. And sometimes, when you copied the picture directly to your PC (Windows) it will miss copied a certain picture(s). U got to be sure by checking the amount of picture(s) / video(s) u have in that specific folder(s).

Yep2. I don't know why it become like this. I hope Apple will fix it in the next update. Its really hard to backup photos now, as i used to copy manually to my PC. Now have to sort everything out, need extra work.
