i5 2500k OC, after 3 weeks got problem

Hi , im not sure if anybody faced the problem before . I managed to oc to 4.5ghz and ran all the prime95 approx 12hrs and it was stable for almost 3 weeks. Temp was around 50-60c while playing games. Switched off all idle mode and standby/sleep etc as well. I dont use my pc 24/7 so i shut them down daily.
But after 3 weeks im facing this problem :
- cpu fan jerking when booting now. - will run slow at start then only reach optimal rpm - now it wont start and auto shutdown and loop over and over again.
What can i do to troubleshoot this? -scythe ashura/i5-2500k/asus-p8p67 pro/only normal ram 4gb
*sorry i got no screenshots to provide , i cant even get into windows now :/

Somehow i managed to get into bios again after reassembled everything ,Ignored the fan speed in bios and i could run the windows fine without fan ( thank god for the huge heatsink still on 40 degrees++).
