1. No it does not. 2. Depends on the skill proc coefficient. Assuming LOH is 100%, the skill proc coefficient may just be 5%, rendering the chance to proc LOH at only 5% on hit. You can Google for the list of all skill proc coefficient.
I think loh mechanics changed already.Its the same whether vs 1 target or large amount of mobs. As long as you do damage, you get that same amount of life as whats on your sheet loh according to your attack speed or the animation of your attacks.
Think of the new loh as life regen, but you need to hit something for it to work.
That being said, loh is for tanky ranged build or melee class.

Since you are using pain enhancer.. Does this process with sentry?
Sentry does not proc LoH as far as i know, or hell not suppose to proc anything but a handful of bugs like chain of torment proccing buriza pierce. Loh only works if you self cast meaning it has to come from u.
With the new Quiver that changes Chakram (Spine of Seething Hatred) to generate Hatred rather than consuming it, it does open up some fun builds.
I tried it with SP - Gloom and I'm able to recover pretty quickly when I spam Twin Chakram, as it hits the most target in term of AoE coverage. It's a nice build for survivability, given that you stay way out of Crowd Control effects, so you can have your Healing effectively put to use in that sense.
Of course, damage and DPS-wise, it pales to M6, but it's a fun build and if you're bored of the 'plant and run' routine.
