In all my previous 3 cases I had never any need to submit any custom form. I just pass over the sirim cert to the custom officer n straightaway he just passed over my phones.

Yellow tint is only temporary. My friend's unit had slight yellow tint at bottom and it is now gone after about 10 days. I believe is similar problem as previous iphone due to glue insufficiently dry when shipped out
