Simple, like all cards (For salary employee)
1) Application form
2) IC
3) Salary Slip
4) Bank statement indicating your cheque deposit/transfer of salary
5) Income Tax documents

Noticed that the free passes to premium lounges only apply to domestic air ports. What about other cards?

Actually the last number is a random number. In credit card numbering system all the numbers are pretty much in sequence except the last so as to fulfill the way luhn formula works.

just wondering why maybank didn't include online transactions in here just like Public Bank did ? still expect us to use mbb amex for it ? hmm but not all online accepted amex
