just got confirmation from my source. pipeline supplying gas to Gas Processing Plant A (the one at the main road side). Happened during midnight.

High-risk, high-reward jobs...

I'm all for high rewards, so I thought a few times of going to work in O&G field a few times because I qualify in arc welding for the pipes. You can get 4-5k a month just welding those pipes, and that doesn't include allowances and OTs.

The high-risk part though... I'm well aware of how careless I am, so the thought perished quickly soon after.

i suspect this is all top management negligence issue.

postpone scheduled turnaround to optimize the production, or delay the repair works when u know dozens of ur pipelines actually have zero thickness.
what can a salarymen do when ur management dont approve and call for repair works?

im not saying this is the actual cause, but just to give just a little idea on how large corporation works.
any corporations for that matter. everyone must obey the whoever seems to be the mastermind.
SOP is SOP. but that can easily be bypassed.
engineers say things as accroding to what the bosses need them to say and engineer the reasonings themselves.

Memang HSE incompetence in general.
Many HSE initiatives not implemented because not familiar with them. Bukan nak berfikir. And management of course want to save costs.

HSE only think like 20 years ago, when the operations now are already advanced.

Any tom & d*** can become HSE personnel. Heck...SPM so terok can become safety supervisor / officer. Really no standard. Many choose safety cos it's the easiest rather than taking technical courses & the mindset 'safety gaji besar'.
What's happening now is many private institutions churn out truck loads of imcompetent safety practitioner for the sake of profit. The only safety people that i respect is those who has technical background / many years of site experience & shift to safety. They have higher understanding of how to do things (doesnt implement ridiculous safety procedures) & easier to deal with.

fyi: I ever met a safety officer with greenbook who doesn't know how to operate microsoft word & excel aged less than 30 yrs lagitewww

Risk Management team failed I guess. Failed to mitigate risks. Even if an abnormality is found/discovered, it still can be operate if proper mitigation steps are put in place
