Before I answer that, a little preliminary info:
1. Never ward alone (I cannot stress this enough). Ask someone to cover you while you ward.
2. It's okay to ward early! I see some pubs scolding others for placing wards too early. This is just wrong. An early ward means two things - you get to see where your enemy places their ward, and you can easily deward/catch them out of position.
3. Runespot wards are critical until the enemy tier 1 towers have been taken, after which you can move on to more aggressive warding (Radiant/Dire nc).
4. The Radiant ancient ward is arguably the most valuable warding spot in the early game. Try your best to keep vision here. Rune-dependent mids will benefit greatly from this.

Now, to answer your question, the following would be bad scenarios to deward:
1. The enemy ward was just placed and you can't see the warder on the map. This means they might still be in the vicinity and may gank you as you attempt to deward.
2. You just lost a war. Leave dewarding to after winning a war. It can be quickly done while you push.
3. Knowledge of the ward can be used against your enemies. If you know a ward is in the area, you may be able to bait a gank from the enemy.

You should have gotten a Desolator after your Drums. Maybe replace your Bottle with an Urn since you didn't go mid and Pudge bought a Bottle already.

Other than that, it's probably the BS who fed them a lot. Going Force Staff and SnY first is terrible.

You have a good mid-game lineup, but there's very little lockdown. Against a team with a decent selection of disables and escape mechanisms, there's not really much you can do. If I had to guess, the biggest mistake was dragging the game up to 45 minutes. PA has a very limited window of functionality, which is up to the point the enemy team picks up an MKB. The fact that she (and Pudge, for that matter) don't have BKBs is a testament to their score as well.

Good lockdown heroes are those with many disabling spells. Lion, Rhasta, ES, etc... Best is Silencer and Doom...

Actually, BH was a decent pick. A mid-game lineup means you'll have plenty of opportunity to gank and farm up track gold. Your team had only one reliable disable (dismember), and since Pudge didn't pick up a BKB against a stun-heavy lineup, that's pretty much down the drain.

Try Clock. His hook isn't too difficult to hit, and the low cd means you can gank often. An excellent pickup if the opposite team has a melee carry.

And Bane

You should get 1 level of your Q, W and E by level 3. After that 9 times out of 10 you max your Q (Battery Assault) first, followed by Rockets and then finally cogs. Always take cogs at level 1.

Aghanims is good on Clock, but you should not rush it. A Clock with Boots and Aghanims is useless compared to a Clock with Boots, Force Staff, Drums and Urn. 12 second ulti is nice, but it will be a very long time before you can start spamming it.

My usual itembuild is Phase > Urn (Bottle) > Drums > Force Staff (Blademail).

Everything after that is situational. You can go straight for Aghanims or you can get Necrobook, Orchid, Hex, BKB or Heaven's Halberd. Just get whatever your team needs the most.

Clock very nice hero , on early + mid game but not so good if late game .
Scepter is a must get items on Clock I think , that 12 scd hookshot is wicked sicked .
Stout shield > Shoes(Can choose between phase/tranquility) > Urn > Wood magic Sticks(not really)/Bottles > Rush for scepter .   
Try to move around and gank after getting boots and Urn , get smoke + TP .
