yes, by poslaju and only to be signed by you and you alone (not even your husband/wife).
last time i asked my wife to apply for this mbb2cards after i found its benefits.
when the card arrives, we were not at home and so it was sent back to the main branch for self-collection.
i went there planning to help her to collect but was not allowed to (with security reason that mbb only allow the owners to collect themselves for credit card package).

but my case is to pick up at poslaju branch, not mbb branch. but poslaju told me that mbb instruction is that for credit card package only the real person allowed to collect.

I have collected some MBB CC from Poslaju , some are very strict, similiar in your case. Some not as strict (can collect on behalf).
It all depends on M'sia Boleh working inside PosLaju, at that time, at that day, at which branch, at which counter (depends on your lucky draw when you get that Q number).

they're phishing you to verify the cards info they've gotten somewhere illegitimately. the more you confirm, the more you're validating the info.

where did you get the cards? by agents or online form (by mbb or 3rd party?) or anyone/where else?

if you can confirm u gotten the cards directly from official maybank offices, then you can screw them kawkaw for disclosing your info to external parties without your explicit consent.

1) when you say cut off date, do you mean your statement date + 20 days?
2) finance charge is usually 1.5% on your outstanding balance, which is correct in your case. late payment charges is when you did not pay within free interest 20 days.

back to your questions:
i) any outstanding balance will be charged 1.5% monthly, so paying before your next statement will be good enough
ii) if you clear all outstanding (1,000+500), of course there will be no finance charges
