Don't be greedy. It's totally not worth it to add additional swing to your attacks but end up being smacked and lose half your HP. Wait for opening, get in, throw away few hits, as the monster is preparing for attack, block/roll the fcuk away and wait for the next opening. Rinse and repeat

Well, there is exception though...

When you are walking on a ledge with fully-charged hammur and a frenzied Tigrex is running towards you from below. You know you gonna get wrecked by that mofo but FCUK THAT. DON'T ROLL AWAY. I REPEAT, DON'T ROLL AWAY. GROW A PAIR AND JUMP OVER THE LEDGE AND DROP THE HAMMER ON ITS FACE AND WATCH THAT MOFO SCREAMING IN PAIN. Feels good mang.
