No, just your pasport. All that you mention are requirement for the japanese visa application previously.

When I use my cc in Japan last year, I never ask for my cc PIN.

Expensive. If more than RM1000 without baggage and other add on not worth it. Like RM1300, add few more hundred can get MAS already.

Should be ok. Their custom is not strict like Aussie.
