did you check inside your box properly. I bought mine from GH too and thought that i didn't receive the 1 month free trial as well. Only after searching properly that i found the voucher

I called up sony regarding the extended warranty.

All you need to do is to go to their website. In the contact us section, drop them a mail about your request to extend your warranty and they will get back to you about how to get it done. From what i understand, they will request for a receipt from the reseller.

In my case, my Ps4 is from GH and they said that as long as i bought it from an authorized reseller/focus store, you qualify for the extended warranty.
