1a. Is it the model code for LTE model, and it means 4G?
1b. If is 4G, how should I check when I purchasing it?

1B : above, model number. Further clarification, Settings > About phone > Model
2. I thought my device have had this issue, turns out it wasn't that bad. So I ignored.

-wall charger
-warranty card [this card YOU are the one to keep, not the shop as the rest of the members hve mentioned the shop has taken/kept it.]
4. if the warranty card has DYNAMIC/THORUS/MIDLAND distribution sticker, chances are it's original. And also, for furhter clarification, Press POWER + VOLUME UP to screen shot, check screenshotted photo if it's smaller than 1920x1080 or 1080x1920
