If everything high/ultra except Mesh Quality, then better off don't put Ultra for settings. Why the fak would people not prioritise Mesh Quality shakehead.gif

Mesh Quality is in other words, Draw Distance, how far you can see before the PC not renders the object.

i.e Low = 1km away, Ultra = can see until 5km away
