Prebuilt NAS is energy efficient as that is one of the thing it was designed for. Custom built you need to research which CPU, MOBO and PSU that will be energy efficient.

As for Plex usage, if you need it to decode first before stream, make sure to get CPU that support hardware decoding. Older CPU does not support hardware decoding for HEVC/h.265
