Vcore spiking 1.6++v, I5 3570k

Recently i noticed my vcore spiking as high as 1.6++v when seen through hwmonitor. I found this problem after i formatted my pc dunno whether got any relation. it seldomly comes out when i monitor 3-4 hours ok most of the time, normally after 1 day can see the vcore max 1.6V++. I set the voltage to be in offset mode, when running prime, voltage stays at 1.208v max. I tried in manual voltage setting also same. stress test only done around 15-30 minutes just to check vcore max. basicly my setup is as below asus p8z77v-lx2 i5 3570k corsair cx500  been googling through the internet, cant say i found a similar problem as this. need your guys help... had you try any other logging tools except this CPUid hardware monitor program?

did u also notice CPUTIN spike to 128c? should BSOD had happened.. HWmonitor got some bug b4 with my rig..but i dun remember which version.... realized it only when owikh told me
