Blank or proto form is normal, for toys and ova/ep 0 or as secondary rider too.
Full bottle technology is not exclusive to sentou himself, probably all 3 organization have access to it. So far there is another 2 using full bottle to transform, Blood Stark and Night Rouge. Not sure both of them is consider monster or Evil Rider.
