2. Upload the filled form HERE

3. Installation appointment date would normally be 2 to 4 weeks after registration. MAXIS processor will contact you within 1 week after order submission to confirm the installation appointment date.

4. If you want to re-arrange your appointment, kindly contact MAXIS Center (1-800-82-1123).

5. Installation done and wait for cash rebate to be credited
