To be honest, I think you're not cut out as a 5th. You mentioned being scared to die, and that's not qualities expected of a good 4/5 position player. Last hitting can be improved with training, and map awareness can be improved with lots of deaths and constant scolding, but the aggressive/passive play in battles is a natural trait that's very hard to change. I've worked with a few teams over the years, and I've seen first-hand the negative effects of a forced role.
Truth be told, most players start DotA with a carry mindset. Don't blame them though, 1st position players gain the most fame.
