There's always the balance between DPS & survivality, and skill built in a different way - there is no one fixed way to make a same skill build.

I think you cared too much about chaos resist, to a point it ruins every other aspect of your char.
You don't need to care much about Chaos Resist unless you've into farming Breach Chaos Boss.
Most people would just leave it as -60%, since chaos damage are easily avoidable, such as poison cloud and chaos projectiles.
So, go find a pair of better rings. What you want to look for are LIFE, resist, and elemental damage with attack skill.

- your life % on tree is OK, except you almost don't have any FLAT LIFE VALUE on gears for it to scale. 200% of 1 is still just 2, you get what I mean?
- Your chest armor is a complete bad news. Zealot Oath convert life regen into ES regen. That is one mod you don't want for LIFE build.
- need a boot with 30% move speed, more elemental resist and LIFE
- Abyss belt is pretty much a meta these days, it have everything you need, plus a jewel socket for Abyss DPS jewel
- not using Abyss Jewels, this is one method that is so easy to push dps these days, get half a dozen of jewel slots on passive for them.

1- uber izaro is small fly    his attacks all can be avoided. I'm not aware he deal chaos damage, majority physical. He can 1 shot people with 5k life if not having enough defense layers.
2- but at the sacrifice of resist and life, rare rings can roll physical & elemental damage anyway
3- if you can keep your flask up all the time, sure. But most people don't want that micro management and cap it 75% with gear, easy. Also to free up flask slot for more important things.
4- yep, flat life value. Armor pieces can goes up to 120+ flat life, ring is around 70 I think, belt depends. Belly is decent.
5- there is. People died from Hardcore Abyss have their item goes to Standard anyway, just that they are much more expensive.

I'm not experienced in melee skills, but here's what I came out with:

Your existing stats shown in Path of Building, numbers might vary a bit in-game:
- total DPS (with sulphur flask up): 73,200
- total LIFE: 2,844

Adjusted passive tree, items, flasks, skill gems:
- total DPS (with atziri & lion roar flask up) 5-links: 133,870 / 6-links: 182,907 <- skill gems aren't max level & quality
- total LIFE (with belly): 4553
^ removed all jewels, there are 3 free jewels sockets
^ added Hatred aura replacing Determination
^ added some crap rare items, obviously there are a lot of rooms to improve on items

Suggest you download Path of Building, import the link above and experiment with it. Compare with your char and see what I changed.

- obviously there are axe out there that are much better if you want to change axe 
- changing into a Kitava's Feast unique axe give 20k more dps for example

Uber lab? Need at least 5k LIFE, Izaro hits over 4k. Although you can avoid most of his attacks if respond quick enough.

make new char do need play from zero?

You want normal iLVL60 normal specific helmet?
In Filtration, do [+ItemRarity] set it to normal, [+Item Level] set it too >= 60, [+Base Item] add "Nightmare Bascinet"

for me i thought of
1) GC/warchief(?) hierophant
2) tectonic strike JUGG
3) SR trickster

anything else i misses that is good for starter build?

not sure but seen people playing that as league starter.

till now I haven't decide which to go.

2) Hierophant - Frostbolts or Glacial Cascade
3) Chieftain - Ancestral Warchief (did Berserker Facebreaker Ancestral Warchief 2 leagues straight, prefer not to)
4) Necromancer - Summon Skeleton
