can you share your benchmark results from (SD Card Tester android app)?

i'm reading 18mB/s write speeds but a dismal 12mB/s read speeds from the web.

Can anyone confirm the above or is it a fluke for the read speed test?

for the first part, confirmed from experience the memory card is going to die.

for the second part, it really sounds like a hardware problem but i wouldn't discount ROM problem if you are using non stock ROM.

Charger and cable if you want. Cable optional for exchange if problem is from charger. Phone need to show them the phone serial number in phone settings about device. Bring receipt for proof of purchase for accessory warranty claim.
