in the character screen, it has 4 sections; 3 skill slots and 1 mutagen slot for each section. when the skill you put matches the mutagen color for that section, you'll get bonus stats. For example, you put 3 ability from melee tree (the one in red) and a red mutagen at the mutagen slot. You'll get 28% attack increase. Same goes for other abilities; sign skills with blue mutagen and potion skills with green mutagen.

yeah, it's from latest patch, and not intended. only workaround for now is to drop the mutagen first then you can craf it again.
