Let me start:

1. accidentally swapped out MH with a 210dps dagger and I was like "how come today the elites so strong one, took so long to kill"
2. teleport to mate standing in 3x firepool, GG in 1.5 secs
3. sold my MH 9xx weapon with emerald sockected/700LoH/2.9%LS/dex/vit (worth 26.5+16.5=43mil). when discover and go back, it was the 1st in the buyback list. means if I ever sell another gear I would forever say bye bye to it.

i just sold my 150m glove to npc yesterday and logged out

210 dex, 74 vit, 73 all resist, 48 crit dmg, 8 crit chance

Well, mine is when patch 1.0.4 hit, i accidentally sold a 80mil zunimassa chest for 8mil. Haha...

tis one everyone sure kena, co-op 4 players. Fren say dun chase goblin.
then I chase golbin then killed by mobs. ( long long time ago when d3 was first released, inferno mobs were powderful ) then when u die, u just watch the goblin jump into his portal.

Oh one more, awesome 2OS manticore was placed at 450mil b/o. Saw it first due to the time difference with EU countries.

Check every hour to calculate similar ones and whether to buy. Saw one slightly better but bid. So decided to go for the bid one. After like 5 hours of thinking, decided to buy the 450m one, well gone. lost the bid also.

And now, end up having to pay 100m more for one similar to the 450m one.

Miss out on lots of awesome amulets too. Hesitation has cost me a lot in D3.

2. Die when trying to revive party members from wall of fire

this one hits home well

Same here . Miss 1 "0". Sold 1.5 mil instead of putting 15 mil. Gone in seconds

2 person DH co-op at a3.. we each die at least 5 times on wall of fire..

1 die..try revive..2 die..1 revive..tp and die..after few times both keep dying tp-ing to each other.. only to die again at next fire slate

not really a facepalm
but i feel idiotic everytime i spam my bear on the walls when i meet waller elites

1 very common facepalm problem is you fight so hard but only found yourself was in typing mode.."12333341323141231..."...

my regular mistake:

get 5 stack then quit game..

1. last time i think still early patch. my first legendary was these justice lantern selling at sky high price, and i was total noob in AH hahaha.. sold it in sec 200k g XD almost quit the game. sad moment.

Accidentally npc vendored my trifecta amulet which is worth at least 40mil.

Accidentally click on my friend's grave while kiting an elite pack. Bam! Dead.

intended to put my tal rasha belt with 10m starting bid, then, when it sold, i got 1m.

rammit i put less 1 zero!

Was kiting using my monk and both my heal skill and health potions had cooldown...wanted to click on serenity to buy time but accidentally clicked vortex skill and brought the whole elite pack to me....BOOM!


another moment before 1.03 patch was out...That was the first time blizzard implemented the Nephalem Valor. I remember I was chasing the damn goblin around the map and without me knowing, I mouse-over my skill bar and dragged a skill out of the screen. I lost my left click skill (FoT) and was cursing like mad cos i lost my NV also.

NV is implemented since the very beginning.

I think you got your facts wrong bro...it wasn't implemented at the very beginning. I can't remember which patch was it but it was definitely not at the beginning.

I am absolutely sure it was there. I still remember many people didn't realize that you can still gain NV even in Hell. The only requirement is level 60.

This is 1 of the more successful feature in D3, has been heavily discussed even in Beta. Some links:
