Hi, I am interested to get the supp card. For the requirement document aka the IC copy. Am I suppose to give the principal card holder IC, or the intended supp card owner IC copy?

Hi, when applying supplementary card, the personal details to be keyed in are supp card holder particulars, right?
I am kinda confused as I am principal card holder applying the supp card for my father. In this case, I should be putting all his particulars (Name, IC, age, income, address etc) instead of mine? |
