Im pretty sure this problem can be solved by changing usb port...

I got 4x front usb port all cant be use for this operation... Change to mobo usb port then weirdly success...

1 thing i can confirm is my front usb port only recognize 1 type of connection (either adb(tab status : in android os) or fastboot(tab status : in bootloader mode)

After change to mobo usb port both type of connection can be detect...

At least thats what happened to me.

Back to bootloader > relock > flash stock firmware  >disable location services > skip adding google account > add vpn > add google account > open playstore > add any CC with us address/phone > success!

I've just made a new purchase with my Google credits. It went thru ok. For the record, I have yet to change my info back to Malaysia address. Still on bogus US address and registered via American express card

Under contact us i email for android apps problem. From there I start to get their reply and so on.

CC must have real address added. Malaysia add also ok. After you do this do not press buy any apps for 24 hours, maybe the system needs this 24 hours to verify your account.

For those who are still waiting for the  OTA update, you can follow below instruction to get quick update.

1. Go to Settings -> Apps -> All

2. Find Google Services Framework.

3. Tap on Force Stop.

4. Select Clear Data.

5. Head back to the Nexus 7’s Settings, go to About, and check for a System Update.

I had to try 4-5 times to finally get the OTA update.
