if every customer is eligible for the form, that would make it almost meaningless... astro subscriptions already covers >50% of all households in Malaysia.

Blue form for astro subscriber can be obtained from all stockbroking firm. I just got some.
Only those astro account holders can apply thru the blue form and if apply one blue form they can apply one normal(White), but cannot have multiple of both
Its already preprinted in the blue form that one have to apply 3000shares, no more no less.
Closing date to be delivered to MIH is 1 October 2012 before 5pm

blue form cannot apply online, have to do the conventional way ie buy bank draft, fill up form, put stamp and drop in at MIH in Kelana Jaya.
White ones can do either manual or online

You have hope and a good chance that why you apply.  Even those who buy 4 digits also have high hope...hehehe
