^You need to be on Nightmare of higher to get this. I took the "A Shattered Crown" -quest, killed Mira with Haedrig, ran to the crypts and made sure I got the checkpoint. Then I checked if there was 4th entrance on top of the 3 crypts you usually have, and if there wasnt, I left the game and resumed. This got me right back to the cemetary where my last checkpoint was and I could quickly check if there was a 4th entrance. It took me about 10 to 15 tries to get it.
