Where is your location when that happens and, Digi?

I had very frequents data disconnect. Any feedback after u revert back to m4? Still got data disconnect issue?
Waiting your feedback because it very frustrated data disconnect everytime i try to stream YOUTUBE around my house area which is kepong. Thank you

The data disconnect issue only happens when i at kepong area but it won't happen when i at office.. That why i thought the disconnect issue mostly because of "YELLOW MAN"..   

its digi issue, i having this problem from iphone till nexus....
if u r not UC and UV, the chances of data disconnect is lesser...

I'm on aokp m5 and franco m3 and yellow man too. No problem of network disconnecting, no camera problem except very difficult to screen shot using the normal power/vol down button.  Out of 10 try, only 1 success. Now I  opted for screenshot under power menu options

I dunno kernel will effect the signal strength. But you can try.

You need install BusyBox which you can get from Play Store or V6 SuperCharger thread on XDA.
Then only you can start initialisation phase.

The rest should be usual, excluding 1 thing which require script manager (If you are lazy guy) Because it won't enable by default due the absence of init.d file.

With Script manager, you can enable /data/99SuperCharger.sh start on boot.

Without Script manager, you need to manually enable it from terminal each time you Reboot.

I think you should try AOSP ROMs, since AOKP based might causing your frequent disconnection problem~
Like CM9, those base on AOSP CM9 ones would be your safe tag.

You don't want go for inspection meh? Maybe is sim card faulty?

baseband should be impossible because 4.0.4 had fix it...
bad configuation as I know you won't go to 4636 do something wacky... (personal judgement)
so better go digi change new sim card, try again. If again, go SME claim warranty said you have problems with connectivity ... vice versa.
