well there is some difference, the difference in difficulty.

the monsters become stronger with more players in the game.

more health, etc.

you can always subscribe to various tunneling services to improve the ping a bit when they become available.

diablo 3 is very playable with no noticeable or game breaking lag at 250ms - 300ms.

the in game ping checking "orb" is still green color when the ping is under 300ms.
