No 3: Just print out the application form you submitted online. I guess many students forgot to download/print out before they submitted it

Those who forget, I am not sure if you still can access it.

For 6 and 11:
Since you can drive to Putrajaya, you can those officers to verify your photocopy documents. Don't forget to bring along your original documents so that they can check it.
For 10:
Get your parent and your professor to be the witness of the Perjanjian. After that, go to the Income Tax Office and ask them to stamp (cop untuk matikan setem) it for you at the top right hand corner of the perjanjian (the stamp is green in colour).
For 8:
You have to visit many people to sign it for you. Read it carefully.

Haha. You can probably wait one year and they might still have not reset your account.
Best if you get down to Putrajaya and ask them to reset it for you personally.
You can print out the EPF statement from the EPF office.
