What conditions are needed for an iPhone 5 to be eligible for the battery replacement program?

I've previously had to replace my iPhone 5 due to the battery expanding pretty badly, and now my replacement phone's battery appears to be on the fritz after one and a half years - Battery Life app diagnostics is stating a 50% battery wear level after 611 charge cycles.
