I will vouch for Kotador's work... used them in the past and i find their guys are very professional.

Tell them to show you the brochure for Classic Moulded Doors, it's got a variety of that design you like and there is a version with a designed timber frame as well if you're into those things.

Ask for Ah Tee when you call Kotador. Nice fella. You may want to ask him to go over to your house as their shop can be a little hard to find unless you're familiar with the area.
