You have to dismantle the plaster syiling for that part only, install the railing, and then, put back.. probably, a new plaster ceiling.
It's a costly approach, but I believe, putting the plaster ceiling back won't be that expensive since all they need to do is screw the plaster board to the ceiling (to it's original rail on the ceiling) and apply the plaster compound..
