It is anything from RM0 to RM20. As mentioned by cklimm, RHB Group offers 4 types of debit card (branded as Cash Connect) and the annual fee is RM20.
As for CIMB Bank debit MasterCard. You can request to open a Basic Savings Account (non-fee) and it comes with an annual fee free CIMB debit MasterCard.
If you are not using CIMB Bank as your primary bank and is ok with limited use of the ATM to withdraw cash, then you should consider to open a
CIMB Islamic Bank Basic Savings Account-i 1. It comes with a
free CIMB Islamic debit MasterCard but you are only entitled to use CIMB Bank ATM to
withdraw cash FOC for 8 times in a month. You will have to pay RM0.53/withdrawal from the 9th and subsequent withdrawals in the same month. The profit rate for this CIMB Islamic Bank Basic Savings Account-i 1 is
0.90% per annum.

There are still a few banks that offer a pure ATM card without debit function such as Alliance Bank.

If you are worried about misuse of your card in the event of theft or the loss of your debit card, you can either deactivate the VISA debit/debit MasterCard function... or for some banks, you can set the card's purchase limit to RM0. That effectively deactivates the retail purchase function of your debit card.
