I can't even get into the game now, the game froze whenever comes to "Dark Souls" title screen..
what crap is this -.-

Same as mine. Try playing it in offline mode.

After I went offline [did not login to PSN], I could access the game.

But weird though, as on the 1st day when I started playing, I could play as normal in online mode.

And then just yesterday, this issue arise.

"when playing Online mode = game freezes whenever comes to 'Dark Souls" title screen', press START button = no respond"

"when playing Offline mode = can play the game as per normal"

how u make weapon+10
my claymore can go to +6 then no option to go+7

Should be able to after you ascend to +6. Normal path right?

i dont know what normal path is but this how it progress
i claymore+5 at reinforce weapon option after reach +5. my claymore gone from the list. then i go modification weapon and i see my claymore can reach 6 but after i done it, its gone from the list

Yea go back to the normal list you can upgrade it there to +10. Modify weapon is just for ascension to +6-10.
