Imo, its happen due to the mechanic/foremen bad habit of placing the nuts on any flat surface of the engine and when the lift the car up, these nuts can move/drop.

Imo, its happen due to the mechanic/foremen bad habit of placing the nuts on any flat surface of the engine and when the lift the car up, these nuts can move/drop.
