as i remember i got this issue in 3gs ios4 with maxis prepaid. screen blank after calling suspected cause by notification by maxis.

if it happened frequently try restore your phone as a last resort.

find the correct name for each icon specified. sometimes it will use different name i.e. Icon-iPhone@2x.png, icon@2x.png, icon.png
even some icons use case sensitive character

afaik, proper scaling for hd icons are 114x114 for app store icons and 118x120 for cydia/system icons.

^u need to install dreamboard.. dreamboard is easy to customize but not sure how it perform in ios 5
