 How to apply STPM

I'm from Chinese Independent School. I'm supposed to take UEC this year, but due to some reasons, I've decided to apply for STPM.

Can anyone kindly tell me how to apply to take STPM? Especially those who have been through before.
I was told to go to jpwpkl. Is it true?
Any forms or documents required for the application?
Is there any deadline?
Besides, can we select the school we want? Or they'll arrange for me?

Reply me asap because I'm having exams right now. And have to apply it right after I got my results since I'm not here during the school holidays.

go jabatan pelajaran daERAH and u will get a form from them , that is the best or u can go mpm website to download the form and it stated there all the things u need to do

If i am not wrong, Chung Hwa Chinese Independent School, KL, offer STPM studies.
Maybe can enquire there?

No. I'm not taking it as a private candidate, but to actually go to a goverment school and study for 2 years.

how old are you?if im not mistaken one of the condition to join form 6 is the age. But, I'm not sure about it tho. Was it before 21 or 20?

your in KL right then go to JPWP.
You could also go to a school that offers form 6 and ask how do you apply.
