Poor defending as a team. The fact that nobody was marking or challenging opponent No. 9 is really a mystery to me.
Next is Fabianski. Yes, the blame should be put onto the defence, but yet again, Fabianski failed in his decision-making (again) when he decided to rush out to challenge the ball and failed. Again, I do not 100% blame Fabianski because I think the overall defence should be blamed but Fabianski really has to improve his decision making or else

Just finished watching MOTD, when Fabianski was charging out, he stopped for a while, then only he jumped and tried to get the ball.
If he had decided to come out and get the ball, just continue charging out. I don't care if he crash with Carroll, maybe that might stop him from scoring or better yet, maybe it's a foul for Carroll

The thing is this is not the first time we faced this problem. How many times we've seen this situation?
We have lots of shot at goal, but opponent had one decent chance,usually from a set piece and usually its a header, and it's a goal. After that opponent defend deep and we cant break them.
It's frustrating really, and it baffles me why we still haven't address this problem until now
