Just to name a few. You need to pay RM10 for stamp duty when applying for OCBC's. Alliance Bank charge RM10 for stamp duty and another RM50 as processing fee if my memory serves me correctly. Not sure if any other banks will charge you at all. I used to have a secured credit card from HSBC and i paid absolutely nothing to them. They did not ask me to pay for the stamp duty either.

Is it once withdrawal of FD, the card is useless and terminated instantly? Stamp duty, meaning they issue a cert to you holding a secured CC. Is there a form for us to fill in to apply for secure CC.

No. There won't be any certificate issued. Every banks have their own set of regulations and do things differently. I am not exactly sure how this thing works at all other banks but at HSBC, they will ask you to sign on a green form (collateral form) and that's it. You do not have to pay for anything at all. No stamp duty and no processing fee.

thanks miles, apparently i receive a CC without any submitting any documents. I thought it's secure card. I only a few k in the saving account from the bank.
