My guess is (no. of remaining un-billed amount x amount of monthly commitment fee)
If you have 4 months to go, and your commitment fee is RM155, then RM155 x 4 months, equals to RM620. Maybe you meed to add something else too.
This is my uneducated guess only. Please check Maxis website :-)

Hay guise.
I sent an used unit of iPhone 4 (bought on April 29, 2011) to Maxis KLCC about two days ago, due to loose part inside the iPhone (there was rattling sounds when I and Maxis reps shake the iPhone).
So they changed the iPhone 1 to 1 with a replacement unit. All settled in 30 minutes sans the usual waiting game before you see those reps.
Several things I observed.
1. They don't know/don't care if I JB the unit. Obviously I sent it in restored to stock.
2. They claimed the replacement unit is indeed new. I don't think so.
3. The replacement unit, OUT OF THE BOX ALSO GOT RATTLING SOUND. Almost similar things happen to my wife's new set.
On April 29 (the day I bought two units of iPhone 4) out of the box, there is no sound. They brought the unit somewhere else, and came back. Suddenly got sound.
Now my wife's unit sometime no sound. When she massages a bit beside the Home Button, the sound comes back. She don't want to change 1 to 1. Malas she said.
So my point is, when you go to replace any unit do check it super meticulously. I got rattling sound from my 1st iPhone 4 (1 month old), I got rattling sound from my 2nd iPhone 4 (several seconds old), my wife got NO sound from her iPhone 4.
Now I'm on my third unit of iPhone 4. So far so good.

Yep. I changed 1 on 1 iPhone several weeks ago. It comes with LL at the end.

I had that ticking (or ratling) sound coming out from my previous iPhone 4. The phone worked OK, but it annoyed the hell out of me.
So I went to Maxis KLCC and got mine replaced with a new (refurbished) set. All settled in one visit :-)
Unfortunately, I lost my expensive Power Support screen protector in the process :-( Re-applied another PS screen protector though.
