a) For lighting, you may want to try the Toilet Bowl Diffuser.. but not so good for distance more than 3m... usually at 5 meters.. i tend to hit direct flash...Here's some tips that I use myself:

b) F2.8... mostly used for creative shots... most of the time F4 to F5.6, to get good DOF...
c) ISO400 used as standard and up to ISO1600 when situation needs, like low light shooting...
d) ISO100-200 & F4-F8 for portraits.. some say can use F11 also, if you have good lighting support, like multiple flash setups..
if you have problem of underexposure due to many white subjects, try Exp +0.3 to +0.7.. (tips from Albnok)
of course, most important of all, refer to Number 1 from shootkk... anticipation and focused, dont wast too much time checking the pics already taken, moment is already lost, just be ready for next one...
