1) banker's check
2) photocopy IC
3) application form
4) Maxis recent Bill???

just called MIH.. was informed that the unsucessful envelopes (for non-electronic application) will be sent out today.
Can check the status by calling them on coming tuesday.. hiaya... sien la... why so slow. |

i'm staying in perak. i already got the return envelope last saturday. i think if you don't receive it by monday... the chances of getting is very high.
