let say we pay by ATM ...which account do we pay to?

the envelop A and B, the address need to be same with your IC. but my IC address is back is penang, while i am staying now in KL. so which address am i suppose to write?

u need to register a CDS account via Securites.

you suppose to put your current address on the form. I called MIH to reconfirm this. They said the address u wrote, is the address that they going to return to you if your application fails.

i did call MIH to reconfirm this, what they told me is the address on FORM A/B is your mailing address, coz that will be the address that they going to mail u back when ur application rejected.

anyone payed by using ATM?..base on the instruction, we just need to attached the receipt from the ATM. but my question is, if rejected, how are they going to refund the $$ since they are not going to open the Envelop A...any idea?

i used PBB ATM, the is this eshare application option or something..can;t remember, key in the amount and the MIH code. Then it will transfer from your saving account, a statement will be printed out from the machine. At the back of the statement, need to fill your details, then attached with the blue form and submit.
