After talking to TNB over the phone (can't find it on the website), this is the process to change name :
1) Get application form from TNB's office
2) Get photostated copy of s&p and ic
3) Get 2x RM10 stamp from post office
4) goto LHDN to stamp
5) go BACK to TNB to submit the form
* Yes, going to TNB office 2x times!

One of the forms need to be signed by TNB officer, thus you will still need to go to their office bbefore LHDN!

I've done my Change Of Tenancy today at TNB office. A tips from above, skip the step 3, no need purchase 'Stem Hasil' from post office, directly go LHDN, paid and they stamp on application form for you. Another warm tips, fill up the forms obtain from TNB before apply for stamp at LHDN ( I've submitted in blank form and had to filled up while at LHDN counter ) After talking to TNB over the phone (can't find it on the website), this is the process to change name :
