Hope someone can help me , thank you

anak yatim piatu means for those who lost both their parents (dad and mum), so i think you cannot put yes.
you no need put the no cukai as your mum didnt work, so not tax payer.
about socso, its PERKESO (Social Security Organisation).

6 & 7 - Your guardians (e.g. your parents). If they're still working, ask for a copy of their salary slip. Or if retired, photostat their "retiree card".
8 & 9 - If you're still in semester 1, I don't think need to do item 8 and 9.
