RM 50 for your usage.

Anything more than RM 50, then you have to pay.
Assuming your usage is only RM 45, you still have to pay RM 50 because that's your commitment fees. You have committed to use RM 50 per month.

If your usage is RM 60, you will pay RM 60.
It is not on top of it, else that will be access fees. I don't think any new plans are offering access fees nowadays, most of them have changed to commitment fees.
