SQL services cant on?
Let me make thing clear before I give u some advice
1) The system, before u intsall NT4, is it a clean server?
2) u install NT and then sql or in server already using NT but u just repair it? and then u install sql?

if posible, just tell me what u have done.

NT4.0 server is just too old
same as my previous company they're afraid to upgrade the server because the software installed *might not* compatible with the new OS
and they cant afford a downtime for that

\device\scsi is definitly the hardware controller
post the full error msg

depends what is connected to SCSI port 0. If you have a tape drive there could have been a timeout error waiting to write data or pull data off the tape. If you have a hard drive I would be concerned if there are multiple occurrences. It may not be an issue with the drive but could be a problem with SCSI chain/controller.
Start off with a reboot of the server at a convenient time to see if it clears it.

The server will shutdown and restart again ..... after 6-7 days , this error will come again .... .

Inside the server have 1 scsi hdd and 1 normal hdd for backup data . and this server run around 4 years ++ , so far so good & stable . but recently this error occur quite often .
